    Pay Range: $66,955.20 - $100,422.40



    Summary:

    The Specialty Clinic Navigator reviews patient referrals and schedules new patient consult appointments. This role collaborates with physicians to identify high risk patient situations and works with patients and family to identify barriers to care and how to remove the barriers. The Specialty Clinic Navigator reviews patient data ensuring requirements for surgery are met.



    Essential Job Functions:



    Reviews patient referrals and schedules appointments with appropriate provider.

    Collaboration with physicians as needed to identify high risk patient situations and takes appropriate action in adjusting patient scheduled appointments.

    Assures all records for patients visit are available including but not limited to imaging and imaging reports, previous procedure notes, therapy notes, etc.

    Manages all surgical patients ensuring requirements for surgery are completed.

    Acts as a resource for prior-authorization team. Assists with patient procedure denials by following up with provider for denial letters and/or setting up peer to peer meetings.

    Performs other duties as assigned or needed to meet the needs of the department/organization.



    Additional specialty training as preferred by the work area:

    Heart & Vascular Division


    Completes specialty care transitional calls to ensure continuity of care, appropriate follow-up, and helps avoid complications or unnecessary hospital readmissions.

    Communicates and works with other areas within the healthcare system to provide streamlined patient flow, provision of services, assures appointments and diagnostic test are scheduled appropriately.

    Participates in patient safety, Joint Commission, and audits for specialty care clinics. Acts as a resource for compliance reporting and accreditation requirements according to established guidelines and within established time frames.

    Certifications:

    Registered Nurse | North Dakota Board of Nursing (NDBON) or Compact State Agreement | Prior to Start Date | HR Primary Sources

    Registered Nurse | Minnesota Board of Nursing | Within 3 Months of Start Date | HR Primary Sources

    Driver's License with Acceptable Driving Record | Driver's License - Current State Licensed | Prior to Start Date | HR Primary Sources

    Basie Life Support (BLS) | America Heart Association | Within 90 Days of Start Date | Learning Management System
